Cognitive Behavioral Therapy for Insomnia (CBT-I) is the most extensively researched treatment for chronic insomnia. It is endorsed as the first-line treatment by the American Academy of Sleep Medicine and the American College of Physicians, and has been shown in multiple meta-analyses to produce lasting improvements in approximately 70 to 80 percent of participants, with results maintained at long-term follow-up.
Unlike medication, CBT-I addresses the actual mechanisms that maintain chronic insomnia rather than simply suppressing symptoms. It is as effective as sleep medication in the short term and significantly more effective over the long term.

CBT-I works by directly targeting the three primary mechanisms that maintain chronic insomnia:
Conditioned Arousal: Through stimulus control techniques, CBT-I systematically breaks the association between the bed and wakefulness that has formed through repeated experiences of lying awake.
Sleep Drive Disruption: Through sleep restriction and consolidation techniques, CBT-I restores the homeostatic sleep pressure that chronic insomnia characteristically undermines.
Dysfunctional Beliefs and Attitudes: Through cognitive restructuring, CBT-I identifies and challenges the catastrophic and inaccurate beliefs about sleep that maintain anxiety and perpetuate the insomnia cycle.

Week 1: Sleep education, sleep diary initiation, establishing baseline.
Week 2: Stimulus control — rebuilding the association between bed and sleep.
Week 3: Sleep restriction and consolidation — restoring sleep drive and sleep continuity.
Week 4: Cognitive restructuring — identifying and modifying dysfunctional beliefs about sleep.
Week 5: Relaxation and arousal reduction — integrating Yoga Nidra and breathwork into the CBT-I protocol.
Week 6: Relapse prevention, maintenance planning, and transition to long-term practice.

A daily sleep diary is the primary data collection tool throughout the CBT-I program. Your sleep diary tracks: time in bed, estimated sleep onset latency, number and duration of nighttime awakenings, final wake time, and sleep quality rating. This data drives the sleep restriction and consolidation calculations and provides objective evidence for cognitive restructuring.
